Select your modem from the list, choose your modem port and indicate whether your phone is tone or pulse.  Then click the right arrow. 

Enter your appropriate dial up number for the phone number, your username and password.  Then click the right arrow. 

Select No for IP address, then click the right arrow.

Enter 63.74.90.21 and 63.74.90.23 for the DNS addresses.  Enter bee.net  for the domain name.  Then click the right arrow.
